1) The patient has been
diagnosed with a disease which results in a life
expectancy of six (6) months or less.
2) The patient's
attending physician and/or hospice medical director
CERTIFIES that the patient has a terminal illness which results in a
life
expectancy of six (6) months or less or IF the illness progresses as
expected.
3) There should be a
primary competent care giver in the home.
Some of the following may be admitting diagnoses:

Cancer (various
sites), Lymphoma

Leukemia, Aids,
Aplastic Anemia
-
End stage heart
disease
-
End stage pulmonary
disease
-
Parkinson's, end
stage Alzheimer's
-
ALS, renal failure,
end stage liver disease
-
End stage CVA/stroke
Levels of Care
-
Routine Care
-
is usually provided in your home (home may also be considered as a
nursing home, assisted living facility, or personal care home)
-
Respite Care -
provided in our Madden Inpatient unit or in a contracted hospital or
nursing home for 5 consecutive days to give the primary caregiver a rest
-
Inpatient General
Care - is provided in our Madden Inpatient unit or in a
contracted facility if the patient chooses
-
Continuous Care -
provided in the patients home by a RN, LPN, CNA,
and other team members for a minimum of eight (8) hours during a
crisis, this time may be divided into increments
